Elevate Pediatrics Utah PLLC (“Elevate Pediatrics,” “we,” or “us”) respects your family’s privacy. This policy explains what information we collect through this website, how we use it, and the choices you have.
What this policy covers
This policy covers this website and the forms on it. It doesn’t cover the medical information you share when your child receives care. That information is kept in your child’s medical record through our secure scheduling and records platform, is protected under federal and Utah law, and is described in our Notice of Privacy Practices, which you’ll receive when you book a visit.
Information we collect
- Information you give us. If you reserve a founding spot, we collect your name, email address, phone number (optional), city, number of children, and your children’s ages.
- Information collected automatically. Like most websites, our hosting provider records basic technical information such as your IP address, browser type, and the pages you visit, to keep the site running and secure.
We don’t use advertising cookies, tracking pixels, or third-party analytics on this website. Please don’t include symptoms or other health information in our website forms or in regular email.
How we use your information
- To respond to you and confirm your founding spot
- To send updates about the clinic opening, location, and enrollment
- To operate, maintain, and protect this website
- To comply with the law
We don’t sell your information, and we don’t share it with anyone for their own marketing.
Who we share it with
We share information only with service providers that help us run this website and our practice, and only as needed for them to do that work:
- Formspree, which processes form submissions and delivers them to us
- Our website hosting provider
- Google Fonts, which delivers the fonts on this site and receives your IP address when the page loads
- ClinikEHR, our scheduling and medical records platform, when you book a visit. Its own privacy terms apply when you use it.
We may also disclose information if required by law, to protect someone’s safety, or as part of a sale or reorganization of our practice.
Children’s privacy
This website is meant for parents and legal guardians. We don’t knowingly collect personal information directly from children under 13. If you believe a child has sent us information, email us and we’ll delete it.
How long we keep it
We keep founding-member sign-up information until the clinic opens and enrollment is complete, or until you ask us to delete it, whichever comes first, unless we need to keep it longer to meet a legal requirement.
Your choices
- Unsubscribe. Every update email includes a way to unsubscribe, or you can email us.
- See, correct, or delete your information. Email us and we’ll respond within 30 days.
Security
We use reasonable safeguards to protect your information, including encrypted connections. No method of sending or storing information online is completely secure, so we can’t guarantee absolute security.

This site links to other websites, such as rater8 and CMS.gov. We aren’t responsible for their privacy practices.
Changes to this policy
If we change this policy, we’ll post the new version here and update the date at the top.
