Legal
Telehealth Consent
This page explains what to expect from a virtual visit with Elevate Pediatrics, so you can make an informed choice. You’ll review and sign this consent in our secure patient portal when you book. As your child’s parent or legal guardian, you give consent on your child’s behalf.
What telehealth is
Telehealth means your child is evaluated and treated by a licensed pediatric nurse practitioner over a live, secure video connection instead of in person.
Benefits
- Care from home, on evenings and weekends
- No travel or waiting room
- Quick guidance on whether your child needs to be seen in person
Limits and risks
- Your provider can’t do a hands-on exam or in-office tests, so some findings could be missed.
- Your provider may decide your child needs in-person care, testing, or emergency care instead.
- Poor video or audio, or a dropped connection, can delay or interrupt a visit.
- We use an encrypted, secure platform, but no electronic system is completely free of security risks.
What we ask of you
- Your child must be physically in Utah at the time of the visit.
- A parent or legal guardian must be present, and your child must be on camera.
- Join from a private, quiet place, and tell us where you are at the start of the visit.
- Share complete and accurate health information, including medications and allergies.
- Please don’t record the visit without asking first. We don’t record visits.
If something goes wrong during a visit
If the connection drops, we’ll try to reach you at the phone number you gave when booking. If your child has an emergency at any point, hang up and call 911.
Talking privately with teens
For teens, your provider may spend part of the visit talking with them privately, which is standard in pediatric care. Confidentiality is handled as permitted by Utah law.
Records and follow-up
- Each visit is documented in your child’s secure medical record, the same as an in-person visit.
- You can request a copy of your child’s records.
- With your permission, we can send a visit summary to your child’s regular provider.
Your rights
- You can ask questions before, during, or after a visit.
- You can decline or stop telehealth care at any time and seek in-person care instead.
- Your child’s health information is protected under federal and Utah law, as described in our Notice of Privacy Practices.
