Terms of Use
These terms apply to your use of this website, operated by Elevate Pediatrics Utah PLLC (“Elevate Pediatrics,” “we,” or “us”). By using the site, you agree to them.
Not for emergencies
If your child may be having a medical emergency, call 911 or go to the nearest emergency room. For a mental health crisis, call or text 988. For a possible poisoning, call Poison Control at 1-800-222-1222. Our virtual visits are not an emergency service.
Website information isn’t medical advice
The content on this website is general information. It isn’t medical advice and doesn’t replace an evaluation by a qualified provider. Using this website, reading its content, or submitting a form doesn’t create a provider–patient relationship. That relationship begins only when a visit is scheduled and accepted.
Who our virtual visits are for
- Children from newborn through age 18
- The child must be physically located in Utah at the time of the visit
- A parent or legal guardian must book the visit and join it
Not every concern can be handled safely by video. If your provider decides a concern needs an in-person exam, testing, or a different level of care, we’ll tell you and help you find the right next step. We may decline or end a visit when telehealth isn’t appropriate.
Fees and payment
Virtual visits are a flat $80 per visit, paid when you book. We don’t bill insurance. On request, we can give you a detailed receipt you may submit to your insurer yourself; we can’t promise your insurer will reimburse you. Medications, labs, imaging, and in-person care from other providers are billed separately by those providers.
Cancellations and refunds
- 24 hours or more before your visit: cancel or reschedule for a full refund.
- Less than 24 hours before your visit, or a missed visit: no refund.
- Same-day bookings: if you book less than 24 hours ahead, you can cancel for a full refund up to 2 hours before your visit.
- Running late: if you join more than 10 minutes late, we may need to reschedule, and the visit counts as missed.
- If your child needs in-person care: if your provider decides at the start of the visit that your child’s concern can’t be safely handled by video and refers you elsewhere, you’ll receive a full refund.
- If we cancel, or our technology fails: you can choose a full refund or a free reschedule.
- If your connection fails: we’ll try to reach you by phone to finish the visit. If we can’t, you can reschedule once at no charge.
To cancel or reschedule, use your booking confirmation or email ask@elevatepedsutah.com. Refunds go back to your original payment method, usually within 5–10 business days.
Because your child must be physically in Utah with a parent or legal guardian present, a visit that can’t go forward for those reasons counts as a cancellation with less than 24 hours’ notice.
If you’re uninsured or not using insurance, you have the right to a Good Faith Estimate of expected charges. Learn more at www.cms.gov/nosurprises.
Prescriptions
Prescriptions are written only when your provider decides they’re medically appropriate, and are never guaranteed. Controlled substances, including many ADHD medications, are subject to additional federal and Utah rules. We manage ongoing ADHD medication for children who already have a diagnosis, and may ask for records from the provider who made the diagnosis. We don’t diagnose ADHD.
Reviews
Reviews shown on this website were collected independently through rater8 from families our provider cared for in pediatric primary care before founding Elevate Pediatrics. Individual experiences vary.
